We accept all insurance plans at Cahaba Medical Care. We do ask that you please bring your insurance card(s) with you to each visit.

In order for us to better serve you, we also ask that you please notify our office of any changes made to your insurance information, phone number and/or address. It is important that we update this information in your file so that we are able to contact you regarding your medical care and can accurately bill your insurance company for your visit.

Navigating Your Insurance with Cahaba Medical Care

At Cahaba Medical Care, our commitment is to provide exceptional medical care for all our patients. We proudly accept all types of insurance. However, a critical aspect of ensuring your visits are covered—even with broad insurance acceptance—is making sure we're listed as your Primary Care Provider (PCP) on your insurance plan.

Why might my visit not be covered?

Your insurance plan may necessitate that specific healthcare providers, like us, are designated as your Primary Care Provider (PCP). If Cahaba Medical Care isn't listed as your PCP, there's a risk that your insurance won't cover your visit, leading to unexpected bills.

What should I do?

  1. Verify with Your Insurance: Before your appointment, contact your insurance provider to check if Cahaba Medical Care is listed as your PCP.

  2. Update Your PCP Details: If we aren't your designated PCP, update your insurance information to ensure seamless billing for your visits.

No insurance?

If you do not have insurance, we offer a discounted sliding scale option. Patients enrolled in our sliding scale program receive physician services at a discounted rate determined by household size and total income. For more information about the sliding scale program and the enrollment process, please contact our office and/or speak to one of our front desk receptionists.

Good Faith Estimate

Cahaba Medical Care will provide care to patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. This estimate will be our best effort in telling you what your total costs would be for expected services, but it would not include any costs that we cannot reasonably predict prior to your visit. This is called a "good faith estimate".

In order to receive a good faith estimate, you can request one at any time ahead of scheduling your appointment. Once an appointment is scheduled, the request must be given at least 3 days ahead of that appointment date.

If you receive a bill from us that is $400 more than the good faith estimate, please contact Cahaba Medical Care for resolution. You will need a copy or a picture of your good faith estimate.

Hardship Program

Cahaba Medical Care offers a hardship program to ensure patients with no or low income have access to the care they need. Eligible patients will qualify for discounted co-pays and procedure fees. Eligibility is based on household size and income and is not limited to uninsured patients.  Patients who are insured, but have trouble affording their co-pay, may also be eligible for a reduced co-pay.

To ensure that you don't receive a full bill, we ask that you please complete your application before your first visit. If you are sick and don't have time to plan for your first visit, you may complete an application afterward; if you qualify for the program, your eligibility will date back 30 days.

You may also call and speak with our Social Work & Outreach Department for more information or to request assistance with your application and/or the required documentation.
