Billing & Insurance
We accept all insurance plans at Cahaba Medical Care. We do ask that you please bring your insurance card(s) with you to each visit.
In order for us to better serve you, we also ask that you please notify our office of any changes made to your insurance information, phone number and/or address. It is important that we update this information in your file so that we are able to contact you regarding your medical care and can accurately bill your insurance company for your visit.
If you do not have insurance, we offer a discounted sliding scale option. Patients enrolled in our sliding scale program receive physician services at a discounted rate determined by household size and total income. For more information about the sliding scale program and the enrollment process, please contact our office and/or speak to one of our front desk receptionists.
Everyone can use a helping hand.
Cahaba Medical Care offers a hardship program to ensure patients with no or low income have access to the care they need. Eligible patients will qualify for discounted co-pays and procedure fees. Eligibility is based on household size and income and is not limited to uninsured patients. Patients who are insured, but have trouble affording their co-pay, may also be eligible for a reduced co-pay.
To ensure that you don't receive a full bill, we ask that you please complete your application before your first visit. If you are sick and don't have time to plan for your first visit, you may complete an application afterward; if you qualify for the program, your eligibility will date back 30 days.

You may also call and speak with our Social Work & Outreach Department for more information or to request assistance with your application and/or the required documentation.
