We accept all insurance plans at Cahaba Medical Care. We do ask that you please bring your insurance card(s) with you to each visit. 

Please also notify the office at any time that your phone number, address, or insurance information changes as this is vital for us to know when we try to contact you regarding your medical care and when we bill your insurance company for your doctor's visit.

No insurance?

If you do not have insurance, we offer a discounted sliding scale option. Patients enrolled in our sliding scale program receive physician services at a discounted rate determined by household income and number of individuals in the house. To participate in the program, please inquire as paperwork must be completed.

hardship program

Cahaba Medical Care offers a hardship program to ensure that patients with no or low income have access to the care they need.  Eligible patients will qualify for discounted co-pays and procedure fees. Eligibility is based on household size and income and is not limited to uninsured patients.  Patients who are insured, but have trouble affording their co-pay, may be eligible for a reduced co-pay.

To ensure that you don't receive a full bill, please complete your application before your first visit. If you are sick and don't have time to plan for your first visit, you may complete an application afterward; if you qualify for the program, your eligibility will date back 30 days.

Completed applications, along with the required documentation, may be turned in to our receptionist. Call and speak with our Social Work & Outreach Department for more information or to make an appointment for assistance with your application or the required documentation.
